Summer School Application

As part of the St Mary's Music School admissions procedure, we require certain pieces of information in order that we can process your application. 5. How can we support you

Please let us know how we can support you, such as mobility, auditory or visual impairments, dyslexia or dyspraxia, or significant behavioural or emotional problems.

6. Data Protection: Photography and filming of children and young people.

Photos/videos may be taken of your child and used by St Mary’s Music School or our printed marketing, electronic marketing, social media and the local and national media. If your child is chosen to perform or be interviewed for publicity purposes with a media organisation, their full name may be used. Throughout the week St Mary’s Music School, media or parents and other attendees may take photos or videos. St Mary’s Music School has no control of photographs or video taken by parents or members of the public and is unable to monitor their usage. I consent to photos/videos being taken of my child by St Mary’s Music School for use in social media in respect of this course.
 

7. Applicant Declaration

If the Applicant is thirteen (13) years or older then the section below must be completed by the Applicant. The School cannot process this application without the applicant’s consent – this is a legal requirement. I understand that as part of my application to the course my parent/parents must provide the School with certain types of personal data about me. I have been given an opportunity to read and understand the St Mary’s Music School's Privacy Notice and I am happy to consent for the time being for my information be used in this way. I also understand that I may withdraw my consent at any time in the future by contacting the School.

8. Parent Declaration

I/We apply for a place at the Short Course for the above-named applicant and, in the event of his/her being accepted, I/we being the person/persons having parental rights in respect of the candidate acknowledge that I/we have viewed a copy of St Mary’s Music School’s Privacy Notice and have read and understood it and I/we consent to the School processing my/our personal data for the purposes of considering the Applicant’s admission to the Short Course. I/We confirm that the information on this application form is correct to the best of my/our knowledge. I/We have given the Applicant the opportunity to read, understand, and consent to the School processing the Applicant’s personal data.

9. Confirmation of a place on the Short Course

Following review of the video submissions, parents of applicants will be contacted by the School to confirm whether or not they have been accepted for a place on the Short Course . Following confirmation of a place, a deposit of £150 will be requested. Please note that deposits are non-refundable except in exceptional circumstances, at the discretion of the Course Leader and School Bursar. The balance will be due 6 weeks prior to the start of the course. Requests for financial assistance should be addressed to the Bursar, Dr JoAnna Collings, bursar@st-marys-music-school.co.uk a minimum of 8 weeks prior to the start of the course. Any assistance given will be means-tested and will be allocated on a needs basis.
